The following instructions applies to the Chinese Visa Application Form V2011A, which is required by all applicants. If you are applying for work visa or student visa, or you are holding a non-US passport, please also submit the supplemental form V2011B. Please note: from 2011/07/01, Houston consulate adopts the new 4-page V2011A form. All previous versions of visa applicaton forms are no longer accepted. Document authentication Tips You do not need to fill out box 4.9 (person who invites you) if you do not have one. You may use self-taken pictures for visa application (please print at photo stores), see visa photo requirement. For US citizens, a 1-year multiple visa is as easy to get as a single visa, and costs the same. Frequent travelers, please make sure you have at least one blank visa page. You may make a single payment to us for consular fee, service fee and shipping.
If you were born in mainland China, Hong Kong or Taiwan, please write you name in Chinese characters on the application form, and you may need to submit additional documents.
